Rogue Image in Admin Panel/User Admin

I have suddenly experienced a bizarre error in my Admin Panel/User Admin. The image for Submissions is no longer the usual one, but a logo for Screen Digest which I'm not using anywhere else on my site and puzzled where it comes from. Also have a broken image for User Groups, but otherwise everything seems normal. Checked my image directories on my server and usual images all present and correct - no sign of any rogue images appearing. Also checked source code of the page and Img tags point to the correct images in Admin/images.

I'm still using PHPFusion 6.00.110 as some Infusions I'm using not yet updated for 6.00.303...

Help - anyone have any ideas what's going on?

Looks like it might be a cookie problem as not visible on my office PC setup.

could be the strange, random and reall rare event, that this image has the same size/checksum and name like the one on the site.
does strg+F5 solve the problem?
